After questions coming up in conversations I wanted to share it here that it is available to other users as well.

Opamp models - I have not used dedicated vendor models, but configured the generic level2 spice model of the UniversalOpamp2 circuit to give an appropriate response for the application.

The line for the 3MHz GBW opamps (MCP6477) used in the filter stages is shown here.

For the input stages I used a 20MHz type (TSV992).

Just be sure to specify the most critical parameters. For this application it is good enough to go with the standard gain and slew rate and mimic the model by adopting the GBW (gain bandwidth).
